Health & Wellbeing Award How to Enter this category Entries can be made in two ways 1) You can nominate yourself or 2) ask someone else to nominate you. Either can be done on this entry form. The judges will identify three finalists from the written submissions. Applicants will be advised if they have been successful as soon as possible after the closing date and a judges visit will be arranged with the three finalists. The visit will enable the judges to determine their winner. Each finalist will receive a certificate and 1 complimentary ticket to the Awards Ceremony on 22nd November 2024 at North Shore Hotel where the winner will be announced.
